How did you? If you did, you should know how you did it.
How DO you? You first must find out IF he was insured. The executor of his estate should have known about any insurance policies he had. That executor also should have filed claims on those policies, which would have paid out any benefits to named beneficiaries. If you don't recall getting a check from a life insurance company since 2009, then you need to find out who the executor of his estate was, and ask them about insurance. Maybe there wasn't any, or maybe you weren't named as a beneficiary.
When my dad died, I was the executor. I filed the claims with the life insurance and IRA custodians, and all beneficiaries were paid directly. They didn't have to do anything, except cash the checks.

If you are/were the named beneficiary and they never paid anyone your money was sitting earning a small percentage each year it goes uncollected. Depending on your state at sometime, ten years, fifteen years, etc. the proceeds go to the states "Uncollected funds account. Try googling your state and uncollected fund. You can file a claim with the State.

You'd have to contact his employer, to see if there was an active policy at the time of his death.
Hint: if he was not employed at the time of his death, he doesn't have a policy through his employer.
Something else you need to know - you can file a claim, on an active policy, but there's no "dibs" on the money. ONLY the named beneficiary gets paid, and the position isn't inheritable. If the named beneficiary died before your dad, or there's no named beneficiary, the money goes to your dad's estate, and must be used to pay off any debts before anyone can inherit.
The legal expenses of reopening the estate, could be considerable. If you're grasping at straws looking for money - don't get your hopes up.

Life Insurance pays the "NAMED" beneficiary only. I suggest you contact his old employer to see if he did have a policy through them and if so who was the named beneficiary.

if he DID have insurance thru work, you should have received a check pretty quick after he died - contact them

It should have all been dulled out in his will that the beneficiary to his estates gets his money/assets. You shouldn't just now be wondering about this 6 years later..

First, contact his company and ask them for the name of the life insurance company. Next, contact the life insurance company and ask for a claim form.
