Are you really sure you won't be in the US for a year?
Most people I know who have traveled extensively, come back to the US from time to time.
Additionally, if something happens do you want to be treated in the US or a 3rd world country?
If it was me, I'd make sure I had US based health insurance that pays for treatment in foreign countries and includes medical evacuation to the US. My ACA compliant health insurance does this. The cost is about $80 more per month, than a similar plan that doesn't include this coverage.

Travel insurance is usually supplemental to health insurance - in the event of a disaster, it will cover local treatment and then a trip home, where you'll need further care. That's where you end up screwed.
