I have Medicare and a supplemental policy. Do I need to buy travel insurance for a trip within Germany? (I'm covered for trip cancellation.)?

Medicare doesn't leave the US and Germany requires that you have coverage to enter the country.
Buy a travel health insurance policy, they tend to be pretty cheap. You'll get a little card to show immigration in Germany.

Yes, if you want any kind of medical coverage while you're out of the country, you need to buy it now, before you leave.

Medicare does not cover anything out of the country.
Some Medicare Supplements do have coverage outside of the country for emergencies only after a $250 deductible and then will only pay 80% up to $50K. The problem here is any follow-up is your responsibility.
Medicare Advantage plans do have coverage for Urgent Care and Emergencies world wide with just your co-pay. However, again any follow-up is your responsibility.
The problem is most providers in foreign countries will expect you to pay first before you get help, even for emergencies. You'll then need to get reimbursement from your Supplement or Advantage plan. This reason alone is enough to warrant getting travel coverage. Also, International policies will pay for more than just emergencies or urgent care.
