if you are at fault your insurance company will pay what they will pay of the coverage you have, if this includes your medical, not that of the other party, they will pay the amount within your coverage
if you want to foot all the bills you can just not report it to the insurance company
if the fault was the other party your insurance company will collect from them

Your question is not clear.
If you was in an car accident, and NOT at fault, and received injuries due to the accident, then you "might" be eligible for an injury settlement from the at fault insurance company.
An injury settlement consists of (reimbursement) of medical bills, any lost wages, then adds some pain and suffering. So, if your own health insurance paid for your medical treatment, then they will expect to be reimbursed for what they paid on your behalf.

Keep in mind if your insurance pays they may want it back from the car insurance claim.

If your personal health insurance paid for the medical bills that arose from the accident that health insurance company is due a refund from the auto insurance company.

You don't get double paid. Your medical insurance carrier would be owed anything you receive that they have already paid for.
