That is the way it works depending on your employers health insurance benefits.

Yes. But if you're out sick long enough, and don't file FMLA, they can fire you which then cancels your insurance.

Health insurance policies do not have the 'waiver of premium 'clause for illness/injury that some other private insurance policies do - such as life insurance coverage to cover your mortgage, as one example.

If you aren't working, there is no payroll from which they would deduct to fund your portion of the premium.
They pay a portion of the premium and so do you; yours just comes out of your paycheck pre-tax. If you aren't getting a paycheck due to an extended absence, you need to pay them your portion.

It generally is not up to the employer, it is up to the health insurance company. Generally, yes you would have to pay it.

Yes. They don't pick up your premiums. That's just the way it is. It may not seem fair, but at the same time why should they pay your portion of your premiums if you're not there doing your job? That's just how it works.

If you want to keep your health insurance while you are sick, you would be required to pay for it.

You have been relieved of duties and not getting any pay until you come back to work. This would apply to any benefits that you receive when working. If you want health insurance you'll have to pay the premiums. It's legal because you are no longer an employee. Should you never return to work how long do you think the employer should pay for your health insurance? No work - no benifits.

No, you can let your premiums laps and pay for your medical care out of pocket.

Yes, they can and they will. Your health coverage needs to be paid regardless if you are on the job or home sick.
