Sounds like a spam email. Check your bak statement and see when the payment went out. The call your Health Insurance Company and check with them. Legimate companies will never email you like this.

So, you call them to confirm receipt of the payment. That form was likely issued prior to their processing your payment.

See if your check cleared, and if not, go into your insurance company and make a payment. Perhaps it was lost in the mail.

Make sure with your bank that the money was taken from your account.
Once you know that you actually made the payment, contact the people with your health plan and let them know that you made a payment and provide the information your bank has provided you.
If that does not settle the issue, dispute the charge with your bank, then once that clears make the payment.

Find proof of payment (bank statement) and contact them back with the proof.

Do some 'troubleshooting' by looking up when you paid the bill, checking to see if the check was processed or if the expense shows on your bank statement, or anything you can find to show you paid the bill. Then call the insurance company's 800 number.
Clearly there has been a mistake here, whether you made it or the insurance company. if you can show they got the money, that you wrote a check and they cashed it, or however it was you paid the premium, then you can show it's THEIR mistake. Otherwise the assumption is that it's YOUR mistake.

If u paid online u open your bank page, find that payment and print it. Or write down all the details - date, time, account number and such. If everything is correct u contact your insurance companh and provide them with data
