What gives you the right to dictate to any doctor how he is to do business? If he is not contracted with an insurance company for certain services, then you don't get them - period. Without paying in cash.

Pretty normal rule. The insurance company likes to know what they're being asked to pay for.
The physician doesn't like to be put in the position of coding something that isn't there. To do so is insurance fraud. Nor does the physician like to be put in the position of being requested to code something s/he has not personally seen.

Pretty standard...they cannot pay a bill without the coding and if the doctor is refusing to code, then they don't get paid.
This puts you, as the customer, in the middle...which isn't fun, but that's the world we live in now.
