My question is if you have time to be posting on the Internet, why are you going to the most expensive place to get medical care? Is this something that is a life/death situation, or should you be going to a more appropriate facility like an urgent care center? With most insurances, the out of pocket is typically much less when you go to the correct facility and stay out of hospitals.
But, to answer your question, it will never show on your insurance billing statement. It will show on your Explanation of Benefits and also on the bill(s) you receive from the different providers you consult with.

copays depend on the insurance coverage you have

There probably will be a copay.
There will be an explanation of benefits saying that you used your insurance at the hospital, what services you received there, how much the insurance paid, and how much you have to pay.

We don't know your insurance company or your policy. Each insurance company has well over 100 different policies. You will have to read what you have.

Most hospitals do not collect the copay on the day of service. They bill you for the copay or whatever amount your insurance doesn't cover.
You will get a bill in the mail. It will show that the hospital billed your insurance company. It will show the balance remaining that you owe.

There will be a copay, for the visit and maybe for some tests they run. And yes the visit will show on the monthly explanation of benefits sent to the primary person on the insurance.

There is always a co-payment. Have you met your annual deductible? I dont know what your question is.
