It is not covered.
Insurance covers plastic surgery or nose jobs (rhinoplasty) only to correct congenital birth defects, to repair damage from an injury or disfiguring disease, or to reconstruct something that was removed because of cancer. It does not cover it if it will improve the appearance of a normal, healthy person.

It won't be covered if it's not medically required due to a problem breathing out of it etc. Cosmetic surgery is not covered with medical insurance.

Covered only to repair damage
If for purely cosmetic reasons it is not covered at all

You need to find an ENT that also does plastic surgery. It's possible that you can get some of it covered by insurance if because of a deviated septum you have an increased number of sinus infections. I had my anesthesia covered, my operating room covered, and other items. What I had to pay for was the surgeon fees because I had to pay to have my nose broken to get straightened. Even though it ended up being somewhat cosmetic, the end result was much better as I could breath much better. AND, people that didn't do the nose break method that I know (so not very scientific) ended up not being fixed.
