It is a subsidized program for getting health care. Some people have a share of cost, some get it free. Penalty for what?

Yes, it is considered coverage. If you have medicaid, then you don't pay the penalty for having enough income to buy insurance but not having insurance. (And if your income is low enough to qualify for medicaid, then it's probably too low for you to have to pay the penalty.)
