Before the ACA (Obamacare), you could in many cases get much cheaper insurance back then, but in many cases with some plans were (discount plans) that was (not) health insurance, or if you had a pre-existing condition, then (no) insurance company would insure you. Or the lifetime limits were too low and if you got cancer, then would go broke.
So when they made the ACA law back in 2010, then all insurance had to comply with certain requirements, which include (no) pre-existing conditions, (no) lifetime limits, plus they added mental health and pregnancy (which upset a lot of people that don’t need it). They also added that some things were free like 1 medical exam per year, some other tests etc. But the problem was when they added all of these things, plus had to cover everyone else healthy or sick, then the cost to insure every-one went through the roof. Now they also will give you help in paying the premiums (if) you qualify.
And yes, with some doctors or providers, they may (not) accept your insurance which might make no sense. In some cases they may have trouble collecting from (that) insurance company, or that the amount the doctor wanted to charge and what the insurance company agreed to pay, was the dispute. So when/if you find an insurance you might want to purchase, in many cases will ask who is your primary doctor and will see if on that insurance company.
Now (unless) you qualify due to a specific event, like losing insurance through work, having a baby etc, then you can’t buy insurance now till late November, to be effective on January 1, 2019. Since you must sign up (only) during open enrollment.

First, it's very expensive for insurance to qualify to use the word "affordable". Any insurance that is allowed to call itself "affordable" probably won't be cheap. So you have to decide whether you want cheap insurance or "affordable" insurance, because it's usual not possible to have both. Being called "affordable" isn't cheap.
Second, no health insurance works for all doctors. Some doctors don't accept any insurance and actually cost more if you have insurance than if you don't.

Health insurance plans work for doctors the same as they work for everybody else. Check Blue Cross/Blue Shield and Aetna.
