Never discuss ailments during a well visit.  That's a good way to get EVERYTHING (including all the lab work from the free column to the you pay column).  Been there and done that.  We always recommend to our clients to set up a completely separate visit.

Of course. That would be a regular doctor's visit. Not a physical.

Yes. If you discuss issues that you already know you have or you ask for (or get) a prescription for anything except birth control, then you will have to pay something.

Discussing any problem during a wellness exam will cause it to be coded as a different kind of office visit, thereby making you liable for the payment.
